We are a 25-bed skilled nursing facility specializing in rehabilitation and returning patients to a lower level of care, more independent, home environment. Each room is equipped with a private shower and patients have access to a large multi-purpose activity room. Our Transitional Care Unit has an outstanding team standing by ready to assist you or your loved one in returning to wherever you may call home. Our remarkable team starts with your physician. Many local doctors follow their own patients in the TCU; however, if your physician does not, we have Hospitalists standing by to assist in managing your care. In addition to our primary care physicians and hospitalists, we have many specialty physicians available for consultations. This is one of the many differentiating features of our TCU. Specialists who frequently consult include Cardiologists, Gastroenterologists, General Surgeons, Nephrologists, Oncologists, Orthopedics, Pulmonologists, and other specialty physicians as needed. Included in the medical team at our TCU is the Director of our Center for Rehabilitation. Specializing in Occupational, Physical and Speech Therapy, our interdisciplinary team, including therapists, nurses, dieticians and your physicians, will collaborate and recommend a course of treatment and therapies aimed at achieving your goals. To be considered for admission, you must have a recent qualifying acute hospital stay (often 3 nights in a hospital) a referral from a physician for skilled nursing services and a recent history and physical completed by the referring physician. Rehabilitation referrals needing admission to our TCU anticipate a length of stay 90 days or less and anticipate discharging to a lower level of care setting when completing their rehab stay.
