Grafton Oaks Rehabilitation & Nursing Center is a 99-bed skilled nursing facility offering short-term rehab and long-term care. We are located in the 18-block historic Grafton Hills neighborhood and have been serving the needs of the community over the last 4 decades. Recently renovated, our building provides a warm and relaxing setting that promotes healing with the comforts of home. Grafton Oaks is the only privately owned and operated skilled nursing facility in Montgomery County, Ohio, giving it a unique home-like environment and approach to person-centered care. Individualized treatment plans are developed for each resident based on their level of care, motivation, and goals. Our team of experienced professionals is dedicated to providing specialized personal care around the clock.
