Whether you or your loved one is coming to us for short-term rehabilitation or for a longer stay for a chronic illness or secure dementia care, you can be assured that Heritage Hills will provide uncompromising care in a comfortable, homelike setting. Our tranquil setting belies the complex care we provide. Working with an interdisciplinary team of physicians, clinicians and therapists, our ultimate goal is to help our short-term patients return home as quickly as possible and to provide optimal health in a relaxing environment to our longer-term residents. For the comfort of hospice patients and their families, we offer a private hospice studio suite. We also house a full studio apartment setting to help ensure that our short-term patients are fully functional prior to their transition home. Our Transition Room is part of our Star Program for smooth discharge, which helps to minimize the chance of rehospitalization after leaving Heritage Hills.
