Those values are: Compassion Unlike other healthcare fields, hospice is only allowed one chance to get it right for our patients and their loved ones. That is why everything we do, we do with compassion. No matter where we join our patients on this journey, we make it a priority to bring compassion. Clinical Excellence Clinical Excellence is our guiding light, we believe that anticipation of patient needs and early interventions lead to quality outcomes. Through evidence based practice and continued education, experienced clinicians provide the highest quality medical care based on the palliative care model and are adaptive to complex patient needs. Furthermore, the expertise and collaboration of our Interdisciplinary Group is focused on a holistic view that integrates body, spirit and mind. Compliance Compliance is the backbone of a hospice program and is taken very seriously at Nevaeh Hospice. Our program operates in compliance with Medicare Conditions of Participation, ACHC Accreditation Standards, NM State Regulation and our Policy and Procedures. This is done while maintaining a very patient centric philosophy of care. Commitment It is our commitment to each patient and their loved ones to honor the individual beliefs and needs. We believe that patients and their loved ones are unique individuals and not a diagnosis or a number. Hospice is a journey with a known expected outcome that is difficult and frightening, however we are also committed to celebrate the life of every patient in the same unique and individual way that we provide their care. Communication The hospice journey can be difficult and scary for both the patient and their loved ones that is why we recognize that communication is one of the most important elements of a successful hospice journey. We strive to focus on communication with both patient and their loved ones with compassion and honesty. We wish to prepare them for their specific disease progression and anticipate possible needs. Collaboration As we join each patient on their individual and unique journey it is key that we collaborate all stages of their journey with them and their loved ones to assure the fundamental principles of hospice; “Neither to hasten nor postpone death” as hospice is about the care at end of life not about ending life Community We believe that as part of the community we must support that community. Through understanding our communities unique and diverse culture makeup, as well as understanding the socioeconomic needs of our citizens.
