The Village at Northrise offers independent living, assisted living, skilled nursing and memory support services. Located on the Village at Northrise campus, Desert Willow I is an award winning 31-bed skilled nursing facility offering post-hospital, short-term rehabilitation and transitional care to our guests. We are honored to have received the AHCA Silver Award and to be 5 star rated overall by the Centers for Medicare & Medicaid Services. We accept Medicare, Medicaid, and most private insurance. Our staff is dedicated to serving the medical and rehabilitation needs of individuals recovering from an illness or injury. Amenities provided for our guests include bedside phones with direct inward dialing, individual televisions with 60 cable channels and WiFi access. Our professional nurses, therapists, Insurance Case Manager, Guest Services Specialist and a Social Worker team up to help our guests navigate the transition from acute care back to community as quickly as possible. Guests also enjoy free transportation to scheduled medical and dental appointments and a wide array of social, educational and spiritual activities and entertainment. Our respite program offers the flexibility for Caregivers to take a much-needed break from the demands of daily life, while providing an opportunity for your loved one to socialize with an array of activities in a safe, supportive, welcoming environment.
