Woodhaven Center of Care Assisted Living Facility is the first Medicaid funded, New York State Licensed Assisted Living Program in Suffolk County. For over three decades, we have been serving our residents and caring for their needs to help them transition into their new phase of life with ease and comfort. Our amenities and services provide residents with everything that they need to continue to be independent while also offering companionship, care and assistance for daily activities. Our staff includes Case Managers, RNs, LPNs, PCAs, HHAs, Nurse Practitioners and a Staff Physician. Primary medical care is available 24 hours a day, 7 days a week, giving our residents and their families the peace of mind that they need knowing that there is always someone there. Residents are under 24-hour supervision and are able to receive meals, housekeeping services, partake in social activities and outings, and other specialized care based on their specific needs. In addition, Woodhaven ALP offers a Suffolk County Aging Office approved respite program. Still not sure if ALP is right for you or your loved one? Assisted Living is suitable for adults who need assistance with one or more of their daily activities such as bathing, dressing, housekeeping, transportation, etc. Our experienced staff is trained to provide appropriate care and services that will help residents to gain independence and flourish in their new stage of life.
